About The Position

This role offers a unique opportunity for nurses to step away from high patient ratios and constant interruptions found in traditional hospital settings. As a Floating Visit Nurse, you will provide one-on-one patient care in their homes, allowing you to build relationships and witness patient progress over time. The position offers significant flexibility, with a primary territory and the ability to float to other areas as needed, ensuring variety, consistent patient volume, and ongoing engagement. Home Health Care, Inc. is a 30+ year Minnesota home care leader focused on supporting nurses so they can prioritize patient care.

Requirements

• Active Minnesota Registered Nurse (RN) license.
• Strong clinical judgment and ability to work independently.
• Comfortable in a field-based, travel-focused role.
• Reliable transportation.

Nice To Haves

• Experience in home health preferred, but not required.

Responsibilities

• Provide skilled nursing visits in homes and community settings.
• Perform assessments, wound care, and medication management.
• Educate patients and families on care plans and recovery.
• Collaborate with RN Case Managers and interdisciplinary teams.
• Float to nearby territories as needed to support census and coverage.
• Maintain accurate, timely documentation.
