About The Position

WesleyLife at Home is seeking an RN with experience in home health, wound care, skilled nursing, infusion therapy, or catheter care to join their team. RNs at WesleyLife help older adults remain independent, confident, and well in their homes. The role involves using clinical expertise and forming meaningful connections to provide personalized care. WesleyLife Home Health offers comprehensive services throughout central Iowa, including medical support, therapy, and assistance with daily activities, delivered in clients' homes. The Visit Nurse (RN) plays a key role in supporting the health, safety, and independence of clients.

Requirements

RN with experience in home health, wound care, skilled nursing, infusion therapy, or catheter care.
Ability to form meaningful connections.
Must complete a pre-hire health assessment and 4-panel drug screen.

Responsibilities

Perform comprehensive nursing assessments, including recertifications, resumptions of care, discharge assessments, and routine in-home visits.
Evaluate client progress, response to treatment, and ongoing needs to ensure high-quality, person-centered care.
Counsel clients and their families on health conditions, care plans, medication management, and other related needs.
Provide clear, compassionate guidance that empowers families and supports safety and independence at home.
Maintain ongoing communication with the Nurse Case Manager regarding updates to healthcare provider orders, changes in client condition, incident reports, etc.
Collaborate with the interdisciplinary team to ensure seamless, coordinated care.
