About The Position

Viva Life Home Health & Home Care is a locally owned agency providing compassionate, high-quality care to patients in the comfort of their homes. We are growing and seeking experienced Home Health Registered Nurses (RN) to join our PRN per-visit team. This position offers flexible scheduling, competitive per-visit pay, and strong clinical support, allowing you to focus on delivering excellent patient care.

Requirements

• Active Texas RN License
• 1+ year home health experience
• OASIS documentation experience required
• Current CPR/BLS certification
• Valid driver’s license and reliable transportation
• Strong clinical assessment and documentation skills
• Ability to work independently in the field

Nice To Haves

• 2+ years home health experience preferred
• Strong wound care experience
• Home health EMR experience
• Excellent patient education and communication skills

Responsibilities

• Provide skilled nursing visits in patient homes
• Perform patient assessments and monitor clinical changes
• Complete OASIS assessments when assigned
• Perform skilled treatments including: Wound care, Medication management, Injections, Catheter and ostomy care, Disease management and patient education
• Communicate patient status with physicians and clinical leadership
• Educate patients and caregivers on care plans and medications
• Complete accurate and timely documentation in the EMR
• Follow HIPAA, infection control, and safety guidelines
