About The Position

The Visit RN provides continuity of care through nursing visits for patients, as delegated by the Case Manager RN who oversees overall care planning and coordination. This role involves delivering nursing care to patients in their homes, adhering to physician prescriptions and agency policies. The Visit RN is responsible for keeping the Case Manager RN updated on patient progress, recommended actions, physician orders, and future visit plans. Individuals in this classification are expected to apply a full working knowledge of nursing principles, demonstrate problem-solving skills, and make independent decisions in complex situations under general supervision. The role may involve strenuous exertion during emergency conditions, such as patient resuscitation in the home, and requires contact with patients in various, unpredictable situations. The position may involve irregular hours and requires a keen awareness of personal safety. Visit RNs also provide assigned on-call support for patients, with per diem Visit RNs participating in on-call rotation voluntarily.

Requirements

  • Associate’s degree required.
  • Minimum of one-year recent experience preferred in home health, acute or long-term care.
  • Current Washington State Registered Nurse license.
  • Current BLS certification required.
  • Valid Washington State driver’s license required.
  • Proof of insurance on automobile used for work required.
  • Driving record in compliance with OMC standards currently in effect or as may be amended, required.

  • BSN preferred.

Responsibilities

  • Provide nursing visits for care of patients as delegated by the Case Manager RN.
  • Provide nursing care to the patient in the patient’s place of residence as prescribed by the physician and in compliance with applicable laws, regulations and policies of the agency.
  • Keep the Case Manager RN informed about patient progress, recommended actions and physician orders, and plans for the next visits.
  • Apply full working knowledge of standards of nursing principles, practices.
  • Demonstrate problem-solving skills and make independent decisions in complex and diversified work situations under general supervision.
  • Provide on-call support as assigned for patients.
