Provides supportive counseling, ADL training, crisis intervention and restorative services (when applicable) to people with psychiatric disabilities in residential programs in compliance with relevant regulations. Manages a rotating schedule at all 24-hour supervised sites, with location of coverage changing based on site need and management. Directly participates in household maintenance activities when required. Provides coverage at all residential sites as needed. Provides training, supervision and assistance to residents in daily activities including food purchases, recreational activities, and meal planning and personal care activities. Assists residents in developing meaningful service plan goals and provides supports to assist residents in achieving their goals. Conducts fire safety training activities and ensures fire drills are conducted in accordance with regulatory guidelines. Participates in all Medicaid billable activities under the leadership of the Supervisor, and/or ensures that all documentation required for the particular program is current and in compliance with regulatory guidelines. Documents interactions with residents in accordance with agency policies and standards. Meets weekly, or as scheduled, with assigned primary residents for counseling and to establish and review service plan goals. Maintains collateral contacts with outside service providers and participates in case conferences as scheduled. Assists residents in procuring and maintaining financial entitlements. Monitors medications as required. Provides 24-hour supervision to residents in congregate facilities. Provides transportation as needed using a personal vehicle when an agency vehicle is not available. Promptly reports all emergencies, incidents, maintenance and safety issues to senior Counselor or Supervisor. Participates in team meetings as scheduled. Completes admission documentation post-intake for new clients. Facilitates sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress. Completes QI Training during onboarding and participates in QI refresher training as needed.
