Provides supportive counseling, ADL training, crisis intervention and restorative services (when applicable) to people with psychiatric disabilities in residential programs in compliance with relevant regulations. Manages a rotating schedule at all 24-hour supervised sites, with coverage determined by site need and management. Provides training, supervision, and assistance to residents in daily activities including food purchases, recreational activities, meal planning, and personal care. Assists residents in developing meaningful service plan goals and provides support for achieving them. Conducts fire safety training and ensures fire drills are conducted according to regulatory guidelines. Participates in Medicaid billable activities and ensures documentation is current and compliant. Documents resident interactions according to agency policies. Meets weekly with assigned primary residents for counseling and to review service plan goals. Maintains collateral contacts with outside service providers and participates in case conferences. Assists residents in procuring and maintaining financial entitlements. Monitors medications as required. Provides 24-hour supervision to residents in congregate facilities. Provides transportation as needed using a personal vehicle when an agency vehicle is not available. Promptly reports emergencies, incidents, and maintenance/safety issues to senior staff. Participates in team meetings. Completes admission documentation post-intake. Facilitates data sharing with individuals to inform partnered decision-making and keep clients informed of progress. Completes QI Training during onboarding and participates in QI refresher training as needed. Cultivates a culture of inclusion and adheres to all safety protocols and procedures.
