At Healthcare in Action, we serve patients experiencing homelessness through an innovative “street medicine” approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems. Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks. The Peer Navigator supports members who are experiencing/ are at risk of experiencing homeless with managing their own health in Anaheim and Garden Grove, CA areas. Assist in motivating behavioral changes in patients to improve health outcomes through education, peer support, housing & resource navigation, and the relaying of shared experiences. The Orange County Float Peer Navigator serves as a flexible regional resource supporting both the Anaheim and Garden Grove Healthcare in Action teams. The primary responsibility of this role is to provide continuity of care and caseload coverage during Peer Navigator leaves of absence, extended vacancies, and other staffing gaps. When coverage support is not needed, the Float Peer Navigator maintains an assigned member panel and performs all core Peer Navigator responsibilities. The Float Peer Navigator may also assist with selected Community Support Partner related functions during extended CSP absences following completion of required training and competency validation. This position requires flexibility, strong organization, sound judgment, and the ability to work effectively across multiple care teams and service areas.
