Requirements

  • High school diploma or GED
  • At least 2 years of experience in healthcare, caregiving, human services, community services, or a related field
  • Strong communication and problem-solving skills
  • Ability to work independently and manage multiple priorities
  • Valid driver's license and car insurance
  • Ability to pass background screening and drug testing

Responsibilities

  • Conduct in-home, virtual, and telephone quality visits
  • Build relationships with clients, families, and caregivers
  • Help resolve concerns and service issues
  • Support positive client outcomes and satisfaction
  • Provide field coaching and mentoring
  • Reinforce training, professionalism, and service standards
  • Help caregivers success in their roles
  • Review concerns, complaints, incidents, and service issues
  • Document findings and recommend solutions
  • Support compliance, quality, and safety initiatives
  • Connect clients with community resources and support services
  • Identify opportunities to improve independence through technology and telehealth solutions
  • Assist with implementing and monitoring innovative support strategies
