Field Supervisor - Home Care
About The Position
This role is an opportunity for individuals with backgrounds in caregiving, DSP, job coaching, case management, community health work, healthcare, or human services to grow into a leadership position. The Field Supervisor will support clients, coach caregivers, conduct quality visits, investigate concerns, and improve service outcomes. Requirements
- High school diploma or GED
- At least 2 years of experience in healthcare, caregiving, human services, community services, or a related field
- Strong communication and problem-solving skills
- Ability to work independently and manage multiple priorities
- Valid driver's license and car insurance
- Ability to pass background screening and drug testing

Responsibilities
- Conduct in-home, virtual, and telephone quality visits
- Build relationships with clients, families, and caregivers
- Help resolve concerns and service issues
- Support positive client outcomes and satisfaction
- Provide field coaching and mentoring to caregivers
- Reinforce training, professionalism, and service standards for caregivers
- Help caregivers succeed in their roles
- Review concerns, complaints, incidents, and service issues
- Document findings and recommend solutions
- Support compliance, quality, and safety initiatives
- Connect clients with community resources and support services
- Identify opportunities to improve independence through technology and telehealth solutions
- Assist with implementing and monitoring innovative support strategies
