About The Position
The Field Registered Nurse (Field RN) administers skilled nursing care to patients on an intermittent basis in their place of residence. Care is provided in accordance with physician orders and the plan of care, under the direction of the Director of Patient Care Services. The role supports patients’ recovery, wellness, independence, and quality of life through personalized home healthcare.
Requirements
- Have a passion for serving and helping others live their best lives possible.
- Graduate from an accredited school of professional nursing.
- Be currently licensed as a registered nurse in the state of the job posting.
- Have a minimum of one year of nursing experience in a home health setting.
- Have current BLS certification.
- Have access to reliable transportation.
Responsibilities
- Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
- Meet daily productivity standards.
- Initiate the plan of care and make necessary revisions.
- Provide services requiring substantial specialized nursing skills.
- Initiate appropriate preventative and rehabilitative nursing procedures.
- Prepare clinical and progress notes for each patient visit and summaries of care conferences for assigned patients in a timely manner.
