We embrace diversity and value individual skills, ideas, and knowledge. This exciting opportunity is for a Field Nurse, Home Care - Per Diem (Per Visit) within the Garden City/Nassau Territory. The role involves practicing in accordance with the American Nurses Association's (ANA) scope and standards of nursing practice, utilizing clinical expertise to plan and deliver safe, therapeutic, and population-specific patient care through assessment, planning, implementation, and evaluation. The focus is on patient and family-centered care, incorporating their unique needs. The Field Nurse will collaborate within a multidisciplinary healthcare team to promote wellness and optimize patient outcomes, providing leadership through orientation, mentoring, and management of team members to ensure compassionate, dignified, respectful, and efficient care delivery. Participation in unit, departmental, and/or institutional performance improvement initiatives is also expected.

Requirements

  • Bachelor's degree in Nursing required for employees hired after 9/1/2020 (those who were hired prior to 9/1/2020 are grandfathered).
  • Current and valid New York State Registered Professional Nurse license.
  • Current and valid BLS certification.
  • Knowledge of standards of practice for all areas of patient care.
  • Excellent communication (oral and written), organizational, customer service, and interpersonal skills.
  • Basic computer skills.

Nice To Haves

  • Bachelors Degree Nursing.
