About The Position

The Field Coordinator & Caregiver Mentor serves as a key support resource for Lifespan’s caregivers, clients, families, and community partners. This role combines field coordination, client and partner outreach, caregiver mentorship, onboarding, quality assurance, and direct care coverage responsibilities to ensure exceptional client service and caregiver success. The position acts as a bridge between caregivers, clients, families, referral partners, and internal teams by conducting field visits, supporting quality assurance efforts, facilitating new employee orientations, participating in outreach activities, and providing direct care coverage when operational needs arise. As a trusted resource and mentor, this individual helps caregivers navigate their first experiences with Lifespan, reinforces company standards and culture, and promotes ongoing engagement and professional growth. This role also serves as a weekday Caregiver Support and Coverage Resource and is expected to provide direct client care coverage Monday through Friday between the hours of 8:00 AM and 4:00 PM when needed to ensure continuity of services and minimize disruptions in care. During periods when direct client coverage is not required, the employee will perform field coordination, caregiver support, orientation, quality assurance, and community outreach responsibilities. This position is highly visible in the community and requires regular travel throughout Santa Cruz County to support clients, caregivers, referral partners, and community outreach efforts. Approximately 40–60% of the role is performed in the field. This is a full-time hourly position. Work schedules may vary based on client needs, field visits, caregiver support activities, orientations, outreach events, and operational priorities. Employees should expect to work approximately 40 hours per week while maintaining flexibility to support the evolving needs of clients, caregivers, and the organization.

Requirements

  • Minimum one year of experience as a Caregiver, CNA, HHA, Personal Care Attendant, or related direct care role required.
  • Valid California Driver’s License, proof of insurance, and access to a reliable personal vehicle required.
  • Ability to travel throughout Santa Cruz County and surrounding service areas; approximately 40–60% of the role is field-based, including client visits, caregiver support, community outreach, and direct care coverage.
  • Ability to safely assist clients with mobility, transfers, and activities of daily living.

Nice To Haves

  • Previous experience in home care, caregiver supervision, field coordination, training, onboarding, community outreach, or staff development preferred.
