About The Position

UnitedHealthcare is hiring a Field Based HSS Coordinator to support its Integrated Care Model. The coordinator relays information about members’ needs, advocates for appropriate care, and helps ensure members receive services that meet their needs. This is a field-based position with a home-based office, serving Hidalgo County, Texas, and surrounding areas. Employees residing within 100 miles of the service area may work an office-based/field schedule. The role involves frequent travel to members’ homes and other community or care settings.

Requirements

• Meet one of the following qualification paths: high school diploma or GED and at least 5 years of experience working with the ABD/SSI population in a clinical health care role; an associate degree and at least 3 years of clinical experience; an unrestricted Texas LPN/LVN license and at least 3 years of clinical experience; or CHW certification and at least 3 years of clinical experience.
• Intermediate proficiency with Microsoft Office, including Word, Excel, and Outlook.
• Have high-speed internet at the residence.
• Have reliable transportation and a valid U.S. driver’s license, with the ability to travel up to 75% of the time in the assigned area to visit Medicaid members in homes or other settings.
• Possess time-management and organizational skills.
• Reside within commutable distance of assigned duties.

Responsibilities

• Complete face-to-face assessments in members’ homes to address unmet needs.
• Travel to meet business needs, including visits within the assigned area and up to 100 miles from the assigned work location.
• Meet documentation and turnaround times based on state contractual requirements.
• Coordinate members’ services.
• Work on service plans.
• Create, edit, save, and send documents, spreadsheets, and emails.
