About The Position

UnitedHealthcare is seeking a Field Based HSS Clinical Coordinator RN to support members in Harris County, Texas, and surrounding communities within commuting distance. The role offers the flexibility to work remotely while carrying out field-based responsibilities, including travel within the assigned territory. The coordinator develops person-centered strategies, connects members with resources, coordinates care, and advocates for members and their families. The position requires adherence to UnitedHealth Group’s Telecommuter Policy. Candidates must pass a drug test before beginning employment.

Requirements

Current unrestricted Texas RN license.
At least 2 years of experience in a community health or healthcare setting.
Intermediate proficiency with Microsoft Office applications.
Reliable transportation, a valid driver’s license, and ability to travel within the assigned territory.
High-speed internet at residence.

Nice To Haves

Experience with Medicaid, LTSS, or long-term care services.
Knowledge of HCBS and integrated care principles.
Knowledge of the principles of most integrated settings, including federal and State requirements such as federal home and community-based settings regulations.
Bilingual in English and Spanish.

Responsibilities

Assess, plan, and implement person-centered strategies.
Connect members with financial, psychosocial, community, and state resources.
Coordinate care across the continuum and serve as a key point of contact.
Advocate for members and families to ensure their needs and preferences are heard and supported.
Collaborate with providers, members, caregivers, and community partners to improve health outcomes.
Travel within the Harris County, TX area and surrounding areas.
