About The Position

Family Health Centers at NYU Langone is seeking a full-time Family Support Worker to join their team in Brooklyn. This role involves engaging families through regular home visits, with the goal of supporting positive parent-child bonding, promoting child and family health and development, and enhancing family resilience. The program operates within a healthcare system and collaborates with internal and external stakeholders. Responsibilities include assessing family strengths and needs, engaging participants in curriculum-based activities, providing case management and advocacy, and making referrals to community partners.

Requirements

  • Bilingual Spanish.
  • A high school diploma/GED with experience assisting children and or families in a community setting.

  • Knowledge of infant and child development.
  • Infant mental health endorsement preferred (if available in the state).
  • Strong listening skills.
  • Ability to identify family strengths and foster self-sufficiency and independence in families.
  • Ability to observe and report accurately on the functioning of individuals and families.

  • Successful parenting and/or child care experience.

Responsibilities

  • Initiating and maintaining regular contact with families, primarily by traveling to their homes.
  • Utilizing curriculum and other education materials to provide service to prenatal families and children under the age of 5.
  • Conducting program promotion and outreach activities in the targeted areas to increase program referrals.
  • Administering standardized assessments with eligible families during home visits.
  • Establishing a trusting relationship with families.
  • Providing prenatal and other health education.
  • Assisting to strengthen parent-child relationships.
  • Assisting parents to improve their skills to optimize the home environment for the child's well-being and safety.
  • Assisting in making and attending health and human service appointments, including activities related to employment and educational goals.
  • Attending role-supporting trainings within the required time frame and on-going professional trainings and workshops on an annual basis.
  • Timely reporting and case documentation in an electronic database management system.
  • Performs other related duties as assigned.
