The Family Support Provider (FSP) Team Leader is responsible for assisting with the delivery of Family Support Services for clients. This role involves identifying potential candidates for recruitment, coordinating family support services throughout CPRC programming and other agency departments, and participating in activities associated with stigma reduction, consumer engagement, and follow-up. The FSP Team Leader will empower parents and caregivers with information about their child's needs, guide them through the planning process, and help them advocate effectively during planning meetings. A strengths-based approach is emphasized, leveraging family strengths for positive outcomes. The role also includes educating families on provider roles, promoting a team approach to care, discussing the benefits of natural supports, introducing problem-solving methods, assisting with transition-aged youth, linking families to community resources, fostering engagement, providing education on available services, and engaging in community outreach. The FSP Team Leader will collaborate as a member of the clinical team, contribute client perspectives, and ensure documentation adheres to clinical policies, DMH guidelines, CARF standards, and federal/state program requirements. Client transportation using agency or personal vehicles is also a responsibility.
