Requirements

Strong foundation in social work.

Responsibilities

Coordinate with a range of providers, such as Department of Social Services case workers, doctors, and schools, ensuring the delivery of comprehensive human services.
Document case management services meticulously.
Offer support in healthcare and educational advocacy.
