About The Position

Responsible professional work delivering family support services utilizing a broad range of community services and natural support systems. Involves a broad range of training and development activities, assessment services, and consultation and requires quality documentation of services provided. Participates in organization’s continuous quality improvement efforts.

Requirements

Associates Degree in human services field and at least one year experience within the Early Childhood, Educational systems, and/or Family Support Services; or a High School education and at least two years’ experience within the Early Childhood, Educational systems, and/or Family Support Services.
Must be Bi-Lingual
Must possess and maintain a valid Florida driver’s license with no record of criminal driving offense of license suspension.
Must possess means of providing job related transportation and show proof of required insurance.
Must be insurable under CHN’s current auto insurance policy.
Must successfully complete and maintain FDLE clearance, Federal background and state criminal background checks, and sexual predator screening.

Responsibilities

Gathers appropriate referral and assessment information for the CSS Team.
Monitors ongoing progress and needs within the family and all supportive services as related to the Matrix. Responsible for entry and integrity of the CBHC CATS program.
Attends Quarterly meetings with CBHC and CHN pertaining to quality assurance KPI/QPI.
Coordinates existing and added services.
Maintains fidelity to the CSS model.
Participates as a member of the CSS Team.
Evaluates effectiveness of Family Support Plan as it relates to the Matrix.
Reviews and evaluates outcomes.
Works within the philosophy and function of Children’s Home Network.
Adheres to virtual office procedures.
Performs other duties as assigned.
