About The Position

Works as part of a two-person team with a Family Support Therapist (FST) in collaboration with the WakeMed Physician Practices- Pediatric practice staff to promote support for positive parenting and family mental health for children ages birth to 10 years, and their families. The team utilizes an integrated model of care delivery while demonstrating respect for the flow of the fast-paced pediatric practice, with the goals of helping kids, helping parents, and helping parents help their kids. The position Is a parenting coach using Triple P and other tools. Serves as a screener and liaison to mental health resources both in and out of the practice, and works to promote engagement, identification, and resolution of pressing social needs as well as care coordination. Works closely with the FST, and supports the team as needed with consultation, interpretation, follow up, logistical support or other needs as identified. Operates within the physical and business environment of the practice, including adherence to practice work hours. Will spend approximately 75% time per week providing direct services to families, as specified above and 25% time to other responsibilities including but not limited to activities related to documentation, data collection, and reporting. The FSC may assist with training, and networking with community resource representatives as needed. Requirements

  • High School Diploma or Equivalent Required
  • 2 Years Healthcare - Ambulatory Care Required
