About The Position

Facilitates patient and family access to appropriate hospital and community resources. This position interacts heavily with both parents and clinical support staff and requires the ability to problem solve in a collaborative manner. The position does occasionally support referrals and work that takes place at the Lehi Miller campus, however that work is done in Epic and doesn’t require being on-site in Lehi to process those workflows.

  • Frequent interactions with providers, colleagues, customers, patients/clients and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
  • Ongoing need for employee to see and read information, labels, documents, monitors, identify equipment and supplies, and be able to assess customer needs.
  • Manual dexterity of hands and fingers to manipulate complex and delicate supplies and equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.

Responsibilities

  • Coordinates with outpatient clinics and inpatient units as part of the Destination Services Team to offer families traveling from out of the area assistance with lodging, transportation, clothing, meal vouchers, and other hospital resources, as needed.
  • Establishing and maintaining interpersonal relationships; developing constructive and cooperative working relationships with others and maintaining them over time.
  • Maintains program materials including wayfinding information, community resources, travel/hotel information, and area attractions.
  • Communicating with people outside the organization, representing the organization to community partners, the public, government, and other external sources to maintain a positive community presence and lasting relationship. This information can be exchanged in person, in writing, or by telephone or e-mail.
  • Leads the Family 2 Family Mentorship Program, which includes leading a steering committee of parent volunteers and staff. Additional duties include recruitment, screening, selection, training, and oversight of parent volunteers. Facilitates outreach efforts, hospital-wide presentations, and program reporting.
