This position implements Health Transitional and Family Services, including eligibility, recruitment, selection, enrollment, attendance (ERSEA), home visits, family needs assessments, referrals, child file reviews, case notes, Family Partnership Agreements, Transitions, collaboration with community agencies, parent education, coordination of MSHS services, family literacy, and identification of community resources. The role involves participating in case reviews to address health, nutrition, and disabilities, managing family services through a case management system, and monitoring recruitment to ensure funded enrollment, including children with disabilities. The position also manages the enrollment process, addresses health and nutrition issues, conducts parent orientations, develops Family Partnership Agreements, and utilizes the Parent, Family, Community, Engagement (PFCE) Framework to offer resources and training. Additionally, the role assists families in accessing community resources, obtains consents for services, prepares clinic paperwork, coordinates with the Child Development Coordinator, maintains a filing system, implements transition processes, collects non-federal shares (In-Kind), ensures timely data entry and reporting, maintains the center parent board, and reports suspected child abuse and neglect. The position requires attending various meetings and performing other assigned duties.
