The Family Services Advocate Worker will implement Health Transitional and Family Services at the Center, including eligibility, recruitment, selection, enrollment, attendance (ERSEA), home visits, family needs assessment, referrals, child file reviews, case notes, Family Partnership Agreements, transitions, collaboration with community agencies, parent education, coordination of MSHS services, family literacy, and tracking of services. This role participates in the Case Review Process to address health, nutrition, and disabilities program areas for children, determining their health, special needs, and nutrition status through file reviews and obtaining current health or dental information. The position assists with managing family services using a case management system, monitoring case reviews to identify family strengths and needs, and providing a support system for families. The role also supports the implementation of a recruitment plan to ensure funded enrollment, including children with disabilities, and manages the enrollment process for families, determining eligibility and priority status. The Family Services Advocate Worker addresses health, nutrition, and disabilities issues, including child exclusions, through consultations, referrals, and home visits when necessary. They conduct parent orientations, coordinate health information, and develop Family Partnership Agreements. Utilizing the Parent, Family, Community, Engagement (PFCE) Framework, the role offers family support, parent training, and encourages family advocacy. The position implements parenting curricula, assists families in accessing community resources, and obtains necessary consents for services. They prepare paperwork for clinics, coordinate with the Child Development Coordinator on various procedures, and maintain an effective filing system for confidential information. The role also manages the transition process for children between programs and educational settings, collects non-federal shares (In-Kind), ensures timely data entry and reporting, maintains the center parent board, and reports suspected child abuse and neglect. The position requires attendance at various meetings and activities and performing other assigned duties.
