About The Position

The Family Resource Specialist (FRS) serves a critical community outreach, referral generation, family engagement, and assessment role within the Healthy Families Indiana program. The FRS is responsible for developing and maintaining referral relationships within the community, actively generating referrals to the program, engaging referred families, completing the validated Family Resilience and Opportunities for Growth (FROG) Scale, identifying family strengths and areas for support, facilitating connection to Healthy Families services, and linking families with appropriate community resources. The FRS develops relationships with hospitals, healthcare providers, the Indiana Department of Child Services (DCS), WIC programs, early childhood providers, schools, social-service organizations, faith-based organizations, and other community partners that interact with expectant parents and families with young children. The FRS represents Healthy Families Indiana in the community and works proactively to ensure potential participants and referral partners understand the program, eligibility requirements, referral process, and benefits of participation. The FRS conducts the FROG Scale objectively and reliably while using a family-centered, conversational, relationship-building approach. The position requires strong interpersonal, communication, outreach, organizational, and writing skills and the ability to establish trusting relationships with families and community partners from diverse backgrounds and experiences.

Requirements

  • Must maintain a valid driver's license, reliable transportation, and automobile insurance when driving is required for job duties.
  • Must maintain confidentiality in accordance with applicable federal and state laws, contractual requirements, and Cornerstone policies.
  • Must comply with mandatory reporting requirements and applicable health, safety, ethical, and professional standards.

Nice To Haves

Responsibilities

  • Actively develop and maintain referral sources to support consistent program enrollment and appropriate utilization of program capacity.
  • Conduct ongoing outreach to hospitals, obstetric and pediatric providers, primary care practices, DCS, WIC programs, public health programs, early childhood providers, schools, social-service agencies, community organizations, faith-based organizations, and other potential referral partners.
  • Educate community partners about Healthy Families Indiana, including program eligibility, available services, benefits of participation, and referral procedures.
  • Establish relationships with key individuals within referral organizations and maintain regular contact to encourage ongoing referrals.
  • Identify and pursue new referral opportunities and community partnerships within the assigned service area.
  • Conduct in-person and virtual presentations, informational meetings, community outreach activities, and other referral-development activities.
  • Represent Cornerstone Community Services and Healthy Families Indiana at community meetings, resource fairs, collaborative groups, and other events as assigned.
  • Distribute approved program information and outreach materials to potential referral sources and eligible families.
  • Follow up with referral partners to strengthen relationships, address referral barriers, and encourage continued utilization of the program.
  • Track outreach activities, referral sources, and referral trends as required.
  • Monitor referral activity within the assigned service area and communicate gaps, opportunities, and emerging referral sources to the Healthy Families Supervisor.
  • Collaborate with program leadership and other staff to implement additional referral-development strategies when referral volume is below program capacity.
  • Maintain positive, professional relationships with community partners and represent Cornerstone Community Services in a manner that strengthens the organization's community presence.
  • Receive and respond to referrals in accordance with Healthy Families Indiana requirements and established program procedures.
  • Initiate timely contact with referred families and use persistent, respectful, family-centered engagement strategies.
  • Explain Healthy Families Indiana services, eligibility, the assessment process, the voluntary nature of services, confidentiality practices, and available supports to families.
  • Establish rapport and trusting relationships with parents and caregivers while respecting individual differences, family structures, cultures, values, and parenting practices.
  • Complete the validated FROG Scale with eligible families in accordance with required protocols and training.
  • Administer the FROG Scale objectively, reliably, and in a conversational, relationship-building manner.
  • Gather information necessary to understand family strengths, protective factors, resources, stressors, needs, and circumstances relevant to services.
  • Ensure assessments are complete, accurate, timely, and consistent with HFA and Healthy Families Indiana requirements.
  • Engage families in discussions regarding identified strengths and opportunities for support in a respectful, nonjudgmental, and strength-based manner.
  • Facilitate a supportive transition from assessment to ongoing Healthy Families Indiana services for families who enroll in the program.
  • Maintain current knowledge of community resources and services available to children and families.
  • Identify resources that may address needs identified through family engagement and assessment.
  • Provide appropriate referrals and connections to community-based organizations and other supports based upon individual family needs.
  • Assist families in navigating barriers to accessing needed services when appropriate.
  • Coordinate with referral sources and community partners consistent with confidentiality requirements and family authorization.
  • Provide appropriate resource information and referrals to families who do not enroll in Healthy Families Indiana when needs are identified.
  • Promote family self-sufficiency and the development of natural and community supports.
  • Complete all required assessment and service documentation accurately and within established timeframes.
  • Enter required referral, engagement, assessment, enrollment, outreach, and service information into designated data systems.
  • Maintain clear, objective, professional, and complete documentation.
  • Ensure documentation supports program requirements, service reporting, quality assurance, and billing when applicable.
  • Track assigned referrals and assessment activity to ensure required follow-up occurs.
  • Maintain documentation of outreach and referral-development activities as required.
  • Respond promptly to requests for corrections or additional documentation identified through supervisory or quality review.
  • Maintain confidentiality of family information in accordance with applicable laws, contractual requirements, and Cornerstone policies.
  • Deliver services in accordance with HFA Best Practice Standards, Healthy Families Indiana requirements, Cornerstone policies and procedures, and applicable contractual requirements.
  • Maintain fidelity to required FROG administration procedures.
  • Participate in record reviews, quality assurance, continuous quality improvement, and other program-monitoring activities.
  • Use supervision to review assessment practice, family engagement, challenging situations, professional boundaries, and opportunities for professional growth.
  • Participate in HFA accreditation, DCS monitoring, program evaluation, and other review activities as required.
  • Identify barriers to family engagement and work collaboratively with the Healthy Families Supervisor to develop appropriate strategies.
  • Maintain required productivity while ensuring the quality and integrity of the assessment and family-engagement process.
  • Participate actively in regularly scheduled supervision in accordance with HFA standards and Cornerstone requirements.
  • Demonstrate willingness to develop reflective capacity, including the ability to consider personal responses, recognize the impact of interactions with others, and use supervision to strengthen professional practice.
  • Be open to feedback, coaching, professional development, and ongoing learning.
  • Use supervision appropriately when encountering complex family circumstances, safety concerns, ethical questions, or situations outside the employee's scope or expertise.
  • Maintain appropriate professional boundaries while developing supportive and trusting relationships with families.
  • Recognize the effects that work with families experiencing significant stress may have on the helping professional and utilize supervision and appropriate supports.
  • Approach families from a family-centered, strength-based, trauma-informed perspective.
  • Demonstrate respect for families regardless of culture, race, ethnicity, language, family structure, socioeconomic circumstances, abilities, values, or life experiences.
  • Recognize and build upon family strengths and protective factors.
  • Avoid judgmental, coercive, or directive approaches to family engagement.

  • Demonstrate humility when working with families whose backgrounds, experiences, beliefs, or circumstances differ from the employee's own.
  • Promote respectful and inclusive services for all participating families.
  • Successfully complete all training required by Healthy Families America, Healthy Families Indiana, Indiana DCS, Cornerstone Community Services, and applicable contractual requirements within established timeframes.
  • Complete required training and demonstrate competency in administration of the FROG Scale and any other screening or assessment tools required for the position before independently performing those functions.
  • Participate in ongoing professional development and maintain competency in family engagement, assessment, infant and child development, family-centered practice, trauma-informed care, cultural humility, community outreach, referral development, community resources, and other areas required by HFA/HFI.
