What You'll Do As a CRNP at Sadler Health Center, you will deliver comprehensive primary care services to patients across the lifespan, partnering closely with physicians, behavioral health specialists, case managers, and medical assistants to provide whole-person care. Requirements

  • Graduate of an accredited Nurse Practitioner program (DNP preferred).
  • Current Pennsylvania State Nurse Practitioner (CRNP) license.
  • Current Registered Nurse (RN) license in the Commonwealth of Pennsylvania.
  • Active Prescriptive Authority License.
  • Valid DEA Controlled Substance Registration Certification (must be obtained prior to start date).
  • Successful completion of Sadler's credentialing and privileging review process upon hire.
  • Minimum of 24 months of experience as a FM provider.

Nice To Haves

  • Experience in a community health, FQHC, or public health clinic setting.
  • Background working with low-income, multi-ethnic, or underserved populations.
  • Proficiency with electronic medical records (EMR) systems.

Responsibilities

  • Provide comprehensive medical assessments and develop individualized treatment plans in accordance with Pennsylvania scope of practice.
  • Diagnose and treat acute and chronic conditions including hypertension, diabetes, depression, anxiety, ADD/ADHD, and behavioral health concerns.
  • Order and interpret appropriate laboratory and diagnostic procedures; coordinate referrals based on patient need and urgency.
  • Coordinate transitions of care for patients moving between inpatient, emergency, and community-based settings.
  • Promote preventive care through routine screenings, immunizations, and well-visit appointments.
  • Educate patients and families about medications, treatment plans, and available community resources.
