About The Position

The THRIVE Youth Program is seeking a Family Physician to join its innovative, interdisciplinary team dedicated to improving the health and well-being of youth and young adults aged 16 to 25. Based in Halifax, the program provides holistic, trauma-informed, and relationship-based care in a welcoming environment where young people are supported to navigate their physical, mental, and social health needs. THRIVE integrates comprehensive primary care services with sexual and reproductive health, gender-affirming care, substance use support, psychotherapy, and mental health services. The program works closely with community partners to reduce barriers to care and ensure youth receive the support they need to thrive. This is a term position funded for 10 hours per week (approximately 0.2 FTE) and remunerated at the provincial sessional rate. The position is currently funded through March 31, 2027, representing an approximately eight-month term commitment. This opportunity offers physicians the chance to provide meaningful, relationship-based care while working alongside an experienced interdisciplinary team committed to supporting young people during a pivotal stage of life.

Requirements

  • Eligible for licensure with the College of Physicians and Surgeons of Nova Scotia (CPSNS)
  • Eligible for certification with the College of Family Physicians of Canada (CFPC)
  • Membership with the Canadian Medical Protective Association (CMPA)

Responsibilities

  • Provide meaningful, relationship-based care.
  • Work alongside an experienced interdisciplinary team.
  • Support ongoing program evaluation by maintaining accurate patient activity data.
  • Track unique patient volumes and the number of patients presenting for care without a valid Health Card Number (HCN).
  • Provide statistics to Nova Scotia Health Medical Affairs upon request.
  • Review billing and service encounter data as part of the program's annual evaluation.
  • Practice through a trauma-informed, culturally safe, and harm-reduction lens.
  • Support youth with complex medical, mental health, and social needs.
  • Foster trusting relationships and create a safe, respectful space for all patients.
  • Advance health equity and reduce barriers to care.
  • Document and report patient activity accurately for program evaluation.
