About The Position

Easterseals NH is hiring a Family Outreach Worker for its Residential Educational Services program. The role provides support and case management to children and families at risk, serving youth who live in the residential program and their families, or youth and families in the greater Manchester area. The worker provides family-centered services in the community, helps families identify resources and strategies, collaborates with residential and clinical teams to support treatment goals, and maintains communication and documentation.

Requirements

• Bachelor’s degree in Human Services or a related field of study.
• At least two years of experience working with children and families.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Provide case management services to youth in the residential program and their families, or to youth and families in the greater Manchester area.
• Work with social services to provide a wide range of family-centered practices, including community-based work.
• Assess families’ community resources and help youth and families identify supportive strategies.
• Work with the residential and clinical team to support clients in achieving their treatment goals.
• Ensure medical appointments are up to date.
• Manage communication with families and collaterals.
• Maintain compliant documentation, including authorizations and reporting.
