MultiCare Behavioral Health is seeking an experienced, compassionate Family Nurse Practitioner to provide psychiatric services through the Greater Lakes Homeless Outreach Stabilization and Transition (HOST) program. The role primarily serves adults ages 18–59 who face serious behavioral health and substance use challenges and may lack consistent access to care. HOST provides field-based, multidisciplinary treatment, including substance use, medical, rehabilitative, and peer services. The position provides psychiatric consultation, evaluation, medication management, and follow-up for community members enrolled in outreach services. The practitioner supports clinical teams and advises urgent care, emergency, inpatient, and primary care providers on medications and strategies for diagnosing and managing behavioral health conditions. Care is based at the Greater Lakes Main Building in Lakewood, Washington, and may also be delivered through community outreach across Pierce County and the South Puget Sound region. This is a 0.8 FTE position (32 hours per week), scheduled Monday through Friday with flexible daytime hours. The practitioner may need to remain on call for staff consultation about patient medical treatment. Driving between clinic, community, patient, and business locations is an essential function; personal or agency-owned vehicles may be used. Greater Lakes Mental Health is part of MultiCare’s Behavioral Health Network and serves more than 10,000 individuals across Pierce County annually. Its services include mental health and substance use disorder treatment for adults, children, and families with Medicaid coverage, through six locations, including a 16-bed inpatient facility. Care is described as recovery-oriented and trauma-informed.
