About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Family Nurse Practitioner for an outpatient primary care opportunity within a collaborative, mission-driven healthcare organization in San Jose, California. This role involves managing an established patient panel while practicing autonomous, relationship-focused medicine. You will join a supportive interdisciplinary care team using EPIC EMR in a stable clinic environment, providing impactful preventative and chronic disease care across diverse patient populations.

Requirements

• Family Nurse Practitioner license in California REQUIRED
• Minimum 2 years of Nurse Practitioner experience REQUIRED
• Primary care experience REQUIRED
• Strong outpatient clinical and relationship-building skills REQUIRED

Nice To Haves

• Experience managing an independent patient panel preferred
• EPIC EMR experience preferred
• Bilingual candidates welcomed but not required

Responsibilities

• Provide full-spectrum outpatient primary care services
• Manage acute and chronic health conditions across the lifespan
• Develop long-term patient relationships through continuity of care
• Perform preventative care, wellness visits, and chronic disease management
• Collaborate with interdisciplinary care teams and support staff
• Maintain accurate and timely documentation within EPIC EMR
