About The Position

A well-established, not-for-profit community hospital is seeking a Family Nurse Practitioner (FNP) to join its outpatient primary care practice in Shelton, Connecticut. This is a full-time, permanent opportunity focused on delivering high-quality, patient-centered care in a collaborative clinical environment. The ideal candidate will have a strong foundation in primary care, preventive medicine, and chronic disease management. Providers will care for a diverse patient population across the lifespan and manage a wide range of conditions, including diabetes, hypertension, asthma, and other acute and chronic illnesses. Both experienced nurse practitioners and new graduates are encouraged to apply. Requirements

  • Master’s or Doctorate degree in Nursing from an accredited program
  • Board Certification as a Family Nurse Practitioner
  • Active Connecticut APRN license (or eligibility)
  • Open to both experienced providers and new graduates
  • Strong clinical judgment, communication, and patient care skills

Responsibilities

  • Provide comprehensive outpatient primary care services, including evaluation, diagnosis, and treatment
  • Perform routine physical exams, preventive screenings, and health assessments
  • Manage acute and chronic conditions such as diabetes, hypertension, asthma, and infections
  • Order and interpret diagnostic tests, including laboratory work and imaging
  • Prescribe medications and develop individualized treatment plans
  • Educate patients on disease prevention, wellness, and lifestyle modifications
  • Collaborate with physicians and multidisciplinary teams to ensure coordinated care
  • Maintain accurate and timely documentation within the EMR system
  • Participate in quality improvement initiatives and patient care programs
