Requirements
- Master’s or Doctoral degree in Nursing from an accredited graduate nursing program with a focus on family practice.
- Current licensures as a Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) in the State of Texas (or eligibility to obtain).
- National certification as a Family Nurse Practitioner (FNP-C or FNP-BC) by a recognized certifying body (e.g., AANP or ANCC).
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications; additional life support credentials as required by EHMC clinical area.
- Demonstrated proficiency in electronic health records, e-prescribing, and use of clinical decision-support tools.
Nice To Haves
- Minimum of one to three years of recent FNP or advanced practice experience in a primary care, ambulatory, or acute-care setting preferred; new graduates with exceptional clinical performance may be considered.
Responsibilities
- Deliver high-quality primary and urgent care, including comprehensive histories, physical examinations, and age-appropriate screenings for patients from infancy through older adulthood.
- Diagnose and manage acute and chronic health conditions (e.g., hypertension, diabetes, minor injuries, infections) and develop individualized, evidence-based treatment plans.
- Prescribe and manage medications, durable medical equipment, and other therapeutic interventions in accordance with Texas law, EHMC policies, and current clinical guidelines.
- Order, perform, and interpret appropriate laboratory tests, imaging studies, and other diagnostics; refine plans of care based on clinical findings and patient response.
- Provide proactive health promotion, disease prevention, and risk-reduction counseling, including immunizations and routine screening programs tailored to the East Houston population.
- Educate patients and families on diagnoses, treatment options, self-management strategies, and lifestyle modifications that support long-term wellness and health equity.
- Coordinate care and make timely referrals to EHMC specialists, ancillary services, and community resources to optimize continuity of care and patient experience.
- Maintain meticulous, timely, and compliant documentation in the electronic health record, reflecting clinical reasoning, interventions, and patient response.
- Collaborate closely with physicians, nurses, case managers, and allied health professionals to support integrated care pathways, throughput, and quality metrics across EHMC.
- Uphold and model EHMC’s standards for patient safety, infection prevention, regulatory compliance, and service excellence in every patient and family encounter.
