About The Position

We are seeking a Nurse Practitioner or Physician Assistant to join our collaborative Family Medicine team in Plainfield, CT. In this role, you will manage your own patient panel and work alongside physicians and other Advanced Practice Providers (APPs). You will be supported by a dedicated Medical Assistant (MA) per provider and a nursing team. The position offers flexibility with two nearby offices for easy coverage and work-life balance. Located conveniently near Providence, RI, and the CT/RI shoreline, this role provides professional fulfillment and opportunities for weekend getaways.

Requirements

Unrestricted State of Connecticut Medical License (or eligible)
APRN (Family, Adult/Geri Nurse Practitioner) or PA training
2+ years’ experience in a primary care or relevant outpatient setting

Responsibilities

Perform routine physical examinations.
Formulate medical, educational, and psychosocial plans to treat patients.
Develop plans for further evaluation of health needs in consultation with physicians and care team members.
Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols.
Identify problems outside of their scope of practice and refer appropriately to a physician.
