About The Position

Hartford HealthCare is seeking a Primary Care Nurse Practitioner or Physician Assistant to join their team in Plainville, CT. The practice offers a strong, stable team dynamic and a supportive work environment. The Primary Care Provider will manage their own patient panel and deliver quality, patient-centered care within a collaborative team. This is a 100% outpatient role focused on Adult Medicine (18+).

Requirements

Unrestricted State of Connecticut Medical License (or eligible)
APRN (Family, Adult/Geri Nurse Practitioner) or PA training
2+ years of experience in a primary care or relevant outpatient setting

Responsibilities

Carry their own patient panel.
Provide quality, patient-centered care.
Perform routine physical examinations.
Formulate medical, educational, and psychosocial plans to treat patients.
Develop plans for further evaluation of health needs in consultation with physicians and care team members.
Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols.
Identify problems outside of their scope of practice and refer appropriately to a physician.
