Primary Care - Nurse Practitioner or Physician Assistant
About The Position
Hartford HealthCare is seeking a Primary Care Nurse Practitioner or Physician Assistant to join our team in Plainville, CT. The Plainville Primary Care practice offers a strong, stable team dynamic and a supportive work environment. The Primary Care Provider will carry their own patient panel and provide quality, patient-centered care among a collaborative team. This is a full-time, weekdays-only position with a 4-day work week and no call required. The role involves performing routine physical examinations, formulating medical, educational, and psychosocial plans, developing plans for further evaluation, and providing appropriate treatment within the scope of practice.
Requirements
- Unrestricted State of Connecticut Medical License (or eligible).
- APRN (Family, Adult/Geri Nurse Practitioner) or PA training.
- 2+ years’ experience in a primary care or relevant outpatient setting.
Responsibilities
- Carry their own patient panel.
- Provide quality, patient-centered care.
- Perform routine physical examinations.
- Formulate medical, educational, and psychosocial plans to treat patients.
- Develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
- Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols.
- Identify problems outside of their scope of practice and refer appropriately to a physician.
