Requirements

• Nurse Practitioner (FNP) or Physician Assistant (PA-C) certification.
• Passion for Primary Care and patient-centered care delivery.
• Interest in building lasting relationships within a community.
• Strong communication skills.
• Collaborative mindset.
• Open to both experienced providers and motivated new graduates.
• Regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 100 pounds.

Responsibilities

• Provide 100% outpatient Primary Care.
• Build meaningful, long-term patient relationships.
• Collaborate with a team of experienced physicians and a skilled PA.
• Participate in a collaborative care model with strong MA and clinic support.
• Engage in a supportive, team-based environment.
• Mentor new colleagues and contribute to a positive team dynamic.
