About The Position

Requirements

Active Indiana NP or PA license in good standing
Current DEA license or ability to obtain
Strong communication and clinical decision-making skills

Responsibilities

Provide patient-centered care focused on hormone replacement, weight management, and targeted wellness.
Conduct patient assessments and develop individualized treatment plans.
Provide follow-up care and monitor patient progress.
Educate patients on treatment plans, wellness, and lifestyle goals.
Build genuine relationships and establish trust with patients.
Explain medical information in a clear, easy-to-understand way.
Collaborate with the Center Director, Medical Director, and clinic team.
Maintain accurate patient documentation and follow established clinical protocols.
Help create a positive, welcoming, high-energy patient experience.
