About The Position
The NP or PA will work in a care team setting in collaboration with other team members including physicians and other allied health staff. The NP or PA will be involved in population management strategies to improve chronic illness outcomes. The provision of care includes but is not limited to comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments, non-pharmacologic therapies, and counseling patients.
Requirements
- Family Nurse Practitioner or Physician Assistant certification.
- Ability to work in a care team setting.
- Experience with population management strategies.
- Experience in comprehensive patient evaluation.
- Ability to order and interpret diagnostic tests.
- Ability to prescribe pharmacologic agents and treatments.
- Ability to provide non-pharmacologic therapies.
- Ability to counsel patients.
