Requirements

• Board Certified Family Medicine Physician REQUIRED
• Minimum 2 years clinical experience REQUIRED
• Active unrestricted Colorado medical license REQUIRED
• Comfortable treating both adult and pediatric patient populations REQUIRED
• Strong commitment to underserved communities REQUIRED
• Mission-driven and collaborative mindset REQUIRED

Responsibilities

• Provide outpatient Family Medicine care for adult and pediatric patients
• Deliver preventive care and chronic disease management
• Maintain patient-centered care within community-focused environment
• Collaborate with interdisciplinary healthcare teams
• Support underserved and uninsured patient populations
• Maintain accurate clinical documentation and treatment planning
