This role delivers direct clinical services, supervises care provided by clinical team members, and partners with leadership to ensure high-quality, compliant, and fiscally responsible care. The Physician actively participates in quality initiatives, care coordination, and organizational programs that advance access to integrated healthcare.

Requirements

  • MD or DO degree
  • Board certification or eligibility in Family Medicine or Internal Medicine
  • Active Indiana medical license or ability to obtain
  • Commitment to serving underserved and diverse populations
  • Strong clinical judgment, communication, and collaboration skills

Nice To Haves

  • Experience in a Federally Qualified Health Center (FQHC) or community health setting
  • Experience with value-based care, population health, or integrated care models
  • Prior experience working in team-based or interdisciplinary environments

Responsibilities

  • Provide direct patient care in accordance with clinical standards and scope of practice
  • Maintain assigned clinic schedule and on-site coverage
  • Participate in equitable call coverage with other providers
  • Deliver care that is patient-centered, evidence-based, and culturally responsive
  • Refer patients within the network when appropriate, while honoring patient preference and clinical judgment
  • Provide supervision and medical direction to clinical staff
  • Collaborate with interdisciplinary teams to support coordinated, team-based care
  • Participate in patient satisfaction initiatives and continuous improvement efforts
  • Support integrated care delivery models across services
  • Participate in quality improvement, utilization review, and patient safety initiatives
  • Support care gap closure and population health management strategies
  • Engage in value-based care programs, including Medicare, Medicaid, and other payer initiatives
  • Contribute to development and implementation of clinical pathways and best practices
  • Ensure compliance with licensing, regulatory, and accreditation standards
  • Support documentation, billing, and payer requirements
  • Provide input on staffing, workflows, and operational improvements
  • Assist with recruitment, onboarding, and mentoring of new providers
  • Participate in Medical Staff meetings, committees, and organizational initiatives
