About The Position
Nazareth Family Medicine, part of Trinity Health Mid-Atlantic, is seeking a Board-Certified/Board-Eligible Family Medicine Physician to support continued primary care expansion and access initiatives across the Philadelphia market. This opportunity offers a 100% outpatient setting, strong clinical support, and the ability to build a patient panel within a mission-driven, community-focused health system.
Requirements
- MD or DO from an accredited institution
- Board Certified or Board Eligible in Family Medicine
- Eligible for Pennsylvania licensure
- Commitment to high-quality, patient-centered care
Responsibilities
- Provide comprehensive primary care for patients across the lifespan
- Diagnose and manage acute and chronic conditions
- Emphasize preventive care and population health management
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Maintain timely and accurate documentation
- Participate in quality and performance improvement initiatives
