About The Position

The Family Medicine Physician serves as the clinical home for a panel of families, building ongoing relationships and providing preventive, acute, and chronic care across the lifespan. The role emphasizes access, health outcomes, and patient trust rather than encounter volume alone. The physician builds and retains a balanced patient panel, closes care gaps, manages acute issues in the clinic, and coordinates specialty and behavioral-health follow-up to reduce avoidable emergency department and hospital use. The physician practices in a collaborative outpatient primary-care setting with nursing and care-coordination support, a modern EHR, and protocols for chronic disease and transitions of care. The role reports to the medical director and includes protected time for panel management and peer review. Compensation reflects panel responsibility and quality as well as encounters.

Requirements

  • MD or DO degree.
  • Board certification or board eligibility in Family Medicine.
  • Active, unrestricted state medical license and DEA registration, or eligibility to obtain both before the start date.
  • Demonstrated outpatient continuity practice managing acute, chronic, and preventive care across the lifespan.
  • Ability to meet quality, access, and documentation standards in an EHR-based clinic.
  • Skill leading a care team and communicating clearly with patients and specialists.

Responsibilities

  • Build and retain a balanced patient panel, providing same- or next-day access for urgent needs and scheduled preventive care that meets evidence-based guidelines.
  • Close care gaps for diabetes, hypertension, and preventive screenings to improve quality metrics quarter over quarter.
  • Manage acute issues in the clinic and coordinate timely specialty and behavioral-health follow-up to reduce avoidable emergency department and hospital use.
  • Complete accurate, defensible documentation in the EHR on the same day to support coding, quality reporting, and continuity of care.
  • Lead nurses, medical assistants, and care coordinators so in-basket messages, refills, and results are handled within agreed turnaround times.
  • Explain care plans clearly and involve patients in decisions they can follow.
  • Participate in panel management and peer review.
