Requirements

• MD or DO from an accredited medical school
• Board Certified or Board Eligible in Family Medicine
• Eligible for New York State medical license
• DEA: Required or eligible
• Open (new graduates and experienced physicians considered)

• Provide comprehensive outpatient primary care services
• Diagnose and manage acute and chronic conditions
• Develop long-term relationships with patients and families
• Promote preventive care and wellness initiatives
• Collaborate with multidisciplinary care teams
