About The Position

The Family Medicine Physician will be responsible for providing comprehensive clinical care to patients. This includes obtaining health histories, performing physical examinations, diagnosing and treating illnesses, and coordinating patient care. The physician will also be involved in patient education, prescription of medication, and meeting documentation standards, including utilizing certified EHR technology. The role emphasizes culturally competent care and involves patients' families as integral parts of the care team. Additionally, the physician may serve as a preceptor for teaching programs and must adhere to HRSA and FTCA regulations.

Requirements

  • Completed residency in primary care specialty.
  • Board Certification is required.
  • Current licensure in the State of Illinois.
  • DEA, Controlled Substance.
  • Maintain CME requirements.
  • Knowledge of applicable rules, regulations and standards relative to quality assurance, safety and infection control required.
  • Electronic Health Records competency.

Responsibilities

  • Obtains health history and perform physical examinations.
  • Diagnoses, treats, counsels, and coordinate care for each patient.
  • Reviews and update problem, and medication list.
  • Meets documentation standards and completes all progress notes within 24 hours following the patient visit.
  • Fulfills on-call responsibilities in accordance with the schedule.
  • Prescribes medication in accordance with Illinois statute and professional practice guidelines.
  • Performs all duties in conformity with the quality standards and clinical guidelines.
  • Meets Meaningful Use standards which is defined as using certified EHR technology to improve quality, safety, efficiency, and reduce health disparities, engage patients and families in their health care, improve care coordination, improve population and public health; all the while maintaining privacy and security.
  • Provides culturally competent care.
  • Recognizes patients’ families as an integral part of patients’ care teams.
  • Ensures that patients’ and families’ needs are met, and clinic policy is followed through.
  • Encourage patients and families to voice questions and/or concerns.
  • Works with patients and their families as appropriate to encourage care self-management.
  • Conducts patient education activities, instructions related to illness and treatment, and reinforcement of provider’s follow-up instructions.
  • Utilizes printed health education materials as appropriate and document all instructions given; while giving consideration to an individual varying level of understanding and adjusts level of presentation to ensure the patient is knowledgeable of medical condition and plan of care.
  • May function as a preceptor in teaching programs including medical residents, medical students, advance practice nurse students as well as other allied health professions.
  • Adheres to Health Resources and Services Administration (HRSA) and Federal Tort Claims Act (FTCA) regulations.
  • Other duties as assigned.
  • Electronic Health Records competency; closing charts and complete billing tabs with appropriate CPT codes.
  • Cultural Competency; ability to provide care and address needs and concerns of patients in alignment with their culture.
  • Interpersonal Skills; ability to effectively work with diverse populations of patients, colleagues and outside agencies.
  • Administrative and organizational skills; ability to organize diverse information.
