About The Position

Community Memorial is seeking a Board-Certified or Board-Eligible Family Medicine Physician to join a well-established primary care practice in Hamilton, New York. The Family Medicine Physician will provide comprehensive care to patients of all ages, serving as the primary point of contact for preventive care, chronic disease management, acute illnesses, and overall health maintenance.

Requirements

MD or DO degree from an accredited medical school.
Board Certified or Board Eligible in Family Medicine.
Eligible for or currently holding an unrestricted New York State Medical License.
Strong clinical judgment and commitment to evidence-based medicine.
Excellent communication and patient relationship-building skills.
Commitment to delivering compassionate, patient-centered care.

Nice To Haves

Current BLS, ACLS, and PALS certifications preferred.

Responsibilities

Provide full-spectrum family medicine services in an outpatient setting.
Diagnose and treat acute illnesses, injuries, and chronic medical conditions using evidence-based medical practices.
Develop individualized treatment plans tailored to each patient's medical history, lifestyle, and healthcare goals.
Conduct annual wellness visits, routine physicals, preventive screenings, and immunizations.
Perform comprehensive patient evaluations and clinical assessments.
Manage chronic conditions through ongoing monitoring, medication management, and patient education.
Coordinate care plans designed to improve long-term patient outcomes and quality of life.
Monitor treatment effectiveness and adjust care plans as needed.
Order and interpret laboratory studies, diagnostic testing, and screening exams.
Review diagnostic findings and consultant recommendations to ensure accurate diagnosis and treatment.
Promote preventive healthcare through education, counseling, and risk-factor reduction strategies.
Coordinate care with specialists, hospitals, therapists, behavioral health providers, and other healthcare professionals.
Facilitate referrals and ensure continuity of care across the healthcare continuum.
Work closely with advanced practice providers, nursing staff, care managers, and support personnel to provide exceptional patient care.
Maintain accurate and timely documentation within the electronic medical record (EMR).
Participate in quality improvement, patient safety, and population health initiatives.
Ensure compliance with organizational policies, regulatory requirements, and clinical best practices.
