About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Family Medicine Physician for an established community health organization in Huntington Park, California. This unique opportunity is based directly on a middle school campus, providing convenient access to primary care for students, families, faculty, and the surrounding community. You'll practice in a mission-driven outpatient setting where preventive care, continuity, and community impact come together every day. The role involves providing comprehensive primary care for pediatric, adolescent, and adult patients, caring for approximately 20-24 patients per day in a well-supported outpatient setting. You will build long-term patient relationships while improving healthcare access within the local community and collaborate with an experienced multidisciplinary healthcare team dedicated to preventive, relationship-based medicine.

Requirements

MD or DO REQUIRED
Board Certified Or Board Eligible In Family Medicine REQUIRED
Active California Medical License REQUIRED
Active DEA Registration REQUIRED
Passion For Community-Based Primary Care REQUIRED

Responsibilities

Provide comprehensive outpatient primary care services
Diagnose and manage acute and chronic medical conditions
Perform preventive care, wellness visits, and chronic disease management
Care for patients across multiple age groups within a school-based community setting
Coordinate specialty referrals and continuity of care
Document patient encounters accurately within the EMR
