About The Position

Comprehensive Full-Lifespan Primary Care: Autonomously lead patient assessments, diagnostic evaluations, and preventative screenings while collaborating with physicians to diagnose and treat complex acute and chronic health conditions. Patient-Centered Education & Coordination: Counsel patients and families on preventive wellness and treatment options while managing direct specialty referrals, community resource coordination, and rigorous electronic health record documentation.
Requirements
- Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
- Nurse Practitioner credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date.
- Registered Nurse credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date.
- Master's degree of Nursing required.
Nice To Haves
- BLS Provider. American Heart Association or American Red Cross accepted.
- Drug Enforcement Admin credentialed from the Drug Enforcement Administration.
- Hands-On Nurse Practitioner work experience is required
