About The Position

This Nurse Practitioner (NP) opportunity is located at Delaware Valley Hospital in Walton, NY and Downsville, NY. The position is full-time, exempt, and seeks both experienced and new graduate Nurse Practitioners seeing all ages. The role involves shared call every 5-6 weeks with one week on. The Nurse Practitioner will provide general medical care and treatment to patients in our medical facility, under the direction of a physician. This includes performing physical examinations and preventive health measures, ordering, interpreting, and evaluating diagnostic tests, and recording physical findings. The NP will formulate plans and prognoses based on patient condition, discuss cases with physicians and other health professionals for comprehensive patient care plans, and may refer patients to physicians or specialized health resources. The FNP must possess current licensure in New York State as an RN and a current New York State license as a Nurse Practitioner.

Requirements

  • Must be board certified and able to be licensed in the state of New York
  • Master of Science in Nursing (MSN) degree
  • Possession of an NPI (National Provider Identification) number, as evidenced by the receipt of a copy of the notification assigning the number
  • Successful completion of an approved Nurse Practitioner program
  • Current NYS Registered Professional Nurse License
  • American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP) certification
  • Current NYS Certified Nurse Practitioner (CNP) license and certification with New York State Education Department
  • Documentation of current CPR certification required within one (1) week of hire, with maintenance of current CPR certification during employment
  • Certified Prescriptive Privilege in NYS

Responsibilities

  • Provide general medical care and treatment to patients in our medical facility, under direction of physician.
  • Perform physical examinations and preventive health measures within prescribed guidelines and instructions of physician.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patient's clinical problems and health care needs.
  • Record physical findings, and formulates plan and prognosis, based on patient's condition.
  • Discuss cases with physician and other health professionals to prepare comprehensive patient care plan.
  • May refer patients to physician for consultation or to specialized health resources for treatment.
