About The Position

Horizon House, a non-profit organization, has been widely recognized for its services as a provider of community-based behavioral health, intellectual disabled and homeless services in Eastern Pennsylvania and the state of Delaware. Each year, Horizon House provides a continuum of services at over 100 sites to over 4,5000 adults with psychiatric or developmental disabilities, drug and alcohol addictions, and/or homelessness. A pioneer in the field of community-based rehabilitation, Horizon House is widely recognized for its innovative approach to the delivery of services in a community setting. Currently, we are seeking a full time Family Living Coordinator / Life Sharing Coordinator to join our Bucks County Developmental Services Division.

Requirements

  • M.A. degree in Human Services/Social Sciences/Healthcare or a related field and at least 1 year of experience in the MR field; or a B.A. Degree and at least 2 years of experience in the MR field; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
  • Knowledge and understanding of Family Dynamics.
  • An ability to maintain complete confidentiality of client, family, and personnel concerns and records.
  • Good analytical skills
  • Superior organizational and communication skills as well as the ability to keep clearly written records and notes.
  • Prior experience in writing programming goals.
  • Ability to work independently and detailed oriented.
  • Must carry beeper and be available for flexible work schedule including evenings and weekends.
  • May be required to attend training to become a certified agency investigator.

Responsibilities

  • Oversee and monitor Life Sharing program.
  • Participate in the interviewing and selecting of prospective families.
  • Coordinate; write and implement comprehensive assessments and service plans with the individual and family in accordance with the State Department of Public Welfare's 6500 regulations.
  • Support relationships in the family.
  • Develop relationships with individuals interested in pursuing Life Sharing situations.
  • May conduct investigations and participate in the agency rotation of on-call investigators.
  • Respond to investigation hotline calls during on-call beeper coverage.
