About The Position

Easterseals PORT Health is seeking an Alternative Family Living (AFL) Provider in North Carolina. As an AFL provider, you will open your home to provide an individual with intellectual and developmental disabilities, and possibly mental health challenges, a unique opportunity to live with a family unit. The supported individuals have varying needs, and the goal is to find the 'right fit' to ensure they have a stable and caring home where they can live, grow, and integrate into the community while working towards goals of daily living skills and independence. The individual may attend a day program or receive other services in addition to the residential services provided by the AFL.

Requirements

  • Must live near/in the Davidson County area.
  • Must be interested in becoming an Alternative Family Living (AFL) Provider in North Carolina.
  • Must be able to provide a stable and caring home.
  • Must be able to support individuals working toward goals of daily living skills/independence.
  • Must be able to provide 24-hour supports and habilitation.

Nice To Haves

  • CNA or DSP Experience is preferred.
