The Family Empowerment Case Manager will focus on empowering family members to achieve greater independence through skill building, development of positive daily routines, strengthening coping skills, and supporting individualized goals that reduce reliance on professional systems and services. This role involves understanding and implementing the Family Empowerment Program’s developmental service model, including the three phases of intervention that require more intensive staff involvement during initial engagement and gradual tapering of services as families build skills, increase independence, and demonstrate the ability to meet their own needs. The Case Manager will hold families accountable to program handbook requirements, expectations, and established program rules while supporting engagement and progress toward individualized goals. They will make day-to-day decisions regarding cases, keep the Assistant Family Empowerment Director informed of high-risk cases and pertinent issues, and provide on-call services as assigned, developing crisis plans for assigned consumers. This position requires implementing evidence-based assessments and interventions, maintaining regular communication with leadership, and modeling trauma-informed, culturally responsive, and strength-based practices. Collaboration with community partners, including CYS, courts, and schools, is essential for continuity of care and service coordination. The role also involves risk and safety management, including timely reporting of critical incidents and maintaining knowledge of protective service laws. Service documentation includes preparing progress reports, completing chart audits, and assisting with outcome data compilation for reporting and funding requirements.
