About The Position
Family Connections Consultant (FCC) to provide a variety of services to support SAGE clients, including conducting home visits, completing assessments with the families engaged in services, and developing service plans pertaining to goals set by DCS. This is an in-person position, but allows for some remote duties. This position allows for a flexible schedule, but must be available nights and weekends, as needed. SAGE has clients throughout the Phoenix Valley!
Requirements
- Bachelor’s degree of Social Work (BSW) or related behavioral health field required
- A minimum of 1 (one) year of human services work-related or internship experience required
- Reliable transportation required, including proof of insurance and clean 5-year driving record; this position may require transportation of children.
- Mileage reimbursement is provided.
Nice To Haves
- Prior experience in In-Home behavioral health services preferred
- Professional documentation and utilization of Electronic Health Record experience highly preferred
- Bilingual preferred, but not required
